Tracking Your Medicines

In order to get the most benefits and to reduce potential risks, it is vital that you take your medications as directed. It is important to keep track of all your medicines—either prescribed or over-the-counter (OTC)—to ensure that you are properly monitoring and managing your medications. 

We have created the following chart to help you keep track of multiple medications as well as their active ingredients, purpose, dose, and prescribing doctor (if relevant). Click to enlarge it so you can easily print it out:

Having an organized chart of your medications will make medication management easier, and will help ensure that you are taking all medications as directed. Be sure to bring the chart to all medical appointments so you can share it with your doctor. Also bring it with you when you go to the pharmacy and ask the pharmacist to review it before picking up a new medication.
